This survey has been designed to help assess the needs of families in the Greater Baton Rouge Area who have children with disabilities between the ages of 2 and 14.   All responses will remain strictly confidential and the results will be used to help inform services and education strategies for children with developmental disabilities in the our local community.
 
Please note the survey asks questions for children in your household who have a disability, not for other children in your care.  This survey will take approximately 15 minutes.
